INFORMATION REQUEST FORM

Once a loan is granted, recipients will be asked to complete the Information Request Form in February of each year they are in school. The information provided helps the Foundation keep its records current.

You will be asked to complete the Information Request Form and return it by a specified date. This can be done in one of two ways.

1. Download the Information Request Form, provide the requested information, then mail the form to the Foundation at the following address:

2. Download the Information Request Form in Microsoft Word format below, provide the requested information, and then e-mail the completed form as an attachment to the Foundation. The e-mail address to use is: .

Completing the Information Request Form is part of your obligation as a loan recipient.
